Your GP Surgery is moving

Please read this information carefully for more details – further information is in the letter enclosed in this link.

As a registered patient with the Melbourne Grove Medical Practice and the Hambleden Medical Practice, NHS Southwark Clinical Commissioning Group (CCG) wrote to you on 13 February 2019 to advise you that your provider from 01 April 2019 would be Concordia Health Limited. (Please note that Concordia has since changed its name to Omnes Healthcare General Practice Limited).You were also advised in the letter that the surgery would be moving into new premises in Dulwich as soon as the building was completed.

We are now writing to update you that from 8am on Monday 25 May 2020 the new address, telephone number and website for your surgery will be:

The new premises are larger, modern and purpose built, and this will allow your surgery to offer a better service by enabling a range of services to be provided. The new premises are a short walk from the Melbourne Grove Surgery and bus numbers 37 and 42 stop on East Dulwich Grove close to the Tessa Jowell Health Centre.

The move means that GP services will no longer be provided at Melbourne Grove or the Hambleden Clinic sites after Friday 22 May 2020. Services will start at the Tessa Jowell GP Surgery from Monday 25 May 2020.

Your medical notes and registration will automatically be securely transferred to the new location of the GP surgery therefore you do not need to take any action.

IMPORTANT INFORMATION RE Covid-19

In line with Covid-19 requirements do not put off contacting your surgery if you need medical help about a new or ongoing physical or mental health issue. But contact the surgery by phone or online first. Only visit the surgery if you are asked to do so.

Parents of babies and young children should also get in touch with their surgery to arrange for their routine immunisations and women who are pregnant or new mothers should contact their surgery if they are worried about their own or their baby or child's health.

Patient Participation Group Dates

You are invited to a patient meeting to find out about the new Melbourne Grove and Hambleden Practice.

We are commencing regular 'Drop in' sessions at the practice for patients to meet one of the senior management team and ask any practice questions. The dates of the next drop in sessions are as per the table below

This modern medical centre is situated in East Dulwich with a team of friendly staff, aspiring to provide the local community with the very highest standard of healthcare.

Our practice is a “community teaching practice” for King’s College London School of Medicine. With the supervision of your doctor or nurse, medical students may, with your consent in advance, join your consultation. This is one way medical students learn to become good doctors and understand patient views about their care. We appreciate your help with their learning. Please let your doctor or practice manager know if you have any questions about our community medical teaching.

Please call after 9.30am and before 6:00pm for any enquires you may have , finding out test results or to register at the practice we will be happy to assist.
